Diabetes is a chronic disease characterized by increased glucose level in the blood. It may affect any person irrespective of gender, ethnicity or age. The complications related to diabetes include nephropathy, neuropathy, retinopathy etc. Diabetes is a major cause of morbidity and mortality in US and causes a significant burden to the health care system. Hence, it requires proper management in order to reduce the cost incurred on healthcare systems and improve the quality of care and life of the patient (Zhou et al., 2020)
In this assessment, the implementation of intervention plan for the patient diagnosed with diabetes will be discussed in detail. I spent 3 practicum hours with Mr. Mathew, a 45 years old male Caucasian patient diagnosed with diabetes for the past 20 years. Special areas of discussion include leadership and change management, nursing ethics, tools applied to educate patient on the intervention plan, communication and collaboration, nursing board and state policies and the impact of intervention in quality of care, safety and cost.
Intervention for Management of Diabetes
Intervention for the management of diabetes should be done in a comprehensive manner involving the patient, family, health care team and relevant state or federal agencies. There are several stages of implementation of the intervention plan. First the patient, Mr. Mathew, will be informed regarding the basics of diabetes, its complications and the management. He will be educated on self-care regarding monitoring of the symptoms. He will also be enlightened on the
current medical advancement that can be used to track the glycemic levels. It will be ensured that due to partial coverage in insurance plan, he gets the medical devices at low costs. Secondly, the plan for care coordination will be discussed with other professionals such as physician, endocrinologists, diabetes care experts, nurses, pharmacists and nutritionists etc. before implementation. The team will ensure the monitoring of the patient on glycemic levels and or unhealing wounds along with regular follow ups. Technology such as medical device, apps or software will also be employed whenever required. Third is the engagement of the healthcare authorities and the state level government, social care organizations, to assist in the financial and community support for the management of diabetes for the patient (Gunter et al., 2021).
Tools Used for Effective Patient Education
To ensure that Mr. Mathew gets all the relevant information on managing diabetes, several methods can be employed given as follows:
Educational Workshops
Conducting educational workshops for physical or virtual interaction can be a useful tool. Similarly, using power point presentations can also be effective in improving the patient education regarding the management and monitoring of the disease. In this way the patients and the families are able to communicate with the doctors and remove confusions concerning the management plan or any misunderstandings that they may have regarding the use of any particular medical device or medical regimen. They are also useful if patient requires to be guided on the use of software or smartphone applications for tracking the health information.
The PowerPoint Presentations may be used to illustrate educational material, images, and videos for the patient’s understanding. The use of medical devices can also be elucidated.
This information can also be provided in written form such as brochures, pamphlets, handouts or flyers for the patient and the family in the course of the workshop or presentation (Jansen et al., 2021).
Physical Demonstration
Physical demonstration accompanied by props or models can also be employed for patient education. The primary purpose of the activity is to allow the participant to practice a particular technique or skill. These methods are used when teaching patient on proper use of devices such as glucose monitoring devices and smart phone apps etc. This reduces the chance of ineffective use of such technology and enhances the efficiency of the intervention plan. The patients and caregivers can use the knowledge acquired to enhance their productivity.
Role of Leadership and Change Management
To ensure success of the care coordination plan, leadership is important for formulation of an effective working relationship and developing channels for communication and interactions. There is need to maintain proper flow of work, address any emerging issues and improve the patients’ care, privacy protection and results (Zhang et al., 2021). Servant leadership and Transformation leadership are vital models that can be employed in improving care delivery since they address the aspects of support and empowering the patients.
